Our goal is to provide the best possible medical care to you/your child in a friendly, efficient and clean environment.  To do so, and to adhere to various insurance requirements, we need your participation.

 

Insurance:  In order for our office to bill your insurance, you will need to bring your current insurance information to every appointment.


Consent to Treat a Minor Child:  A legal guardian must be present at the initial office visit for any child under 18.  If another caregiver, such as a nanny or grandparent, brings the child for the appointment, they should bring a signed consent from a parent to permit exam and treatment.

 

Contact lens wearers:  Patients who wear contacts should come in wearing their lenses to each appointment.  If the lenses were prescribed elsewhere, bring the prescription details with you.

 
Food and Drink:  In order to maintain a clean and healthy environment for all our patients and staff, we do not permit food or drinks to be brought into our waiting room or exam rooms.  This holds for both children and adults.

 

Cell Phone Use: Please limit use to essential conversations and turn your phone to silent during the length of your visit.
 
Missing Appointments:  When you schedule an appointment for yourself or your child, that time is held for you.  If you do not show for the appointment, you have denied the time to another patient who could have been seen.  We request that you give a minimum of 24 hours notice to cancel or change an appointment.  Appointments cancelled with less than 24 hours notice, or missed without notice, will be charged $35.00.  After 3 missed appointments, the office may not make further appointments for your child.  If that happens, the office will forward your child’s records to a doctor of your choice upon receipt of a signed release of information request.


Changing Diapers:   In order to provide a clean, healthy and comfortable environment for all our patients, we ask that you do not change your child's diapers in our examination rooms.  Changing stations are located in the men’s and women’s bathrooms on the same floor as our office.

 

Pets:  Pets are not permitted in the office at any time, unless it is a service animal
